During your dental exam, Dr Dena or one of our hygienist will clean your teeth and perform a thorough examination of your teeth and gums .   Regular Cleaning

  To prevent gum disease Gum (periodontal) disease is a bacterial infection caused by plaque – the sticky, colorless, bacteria-filled film that adheres to your teeth. As plaque builds up on teeth, it hardens and becomes tartar, which can be difficult to remove. The bacteria in plaque produce toxins that irritate the gums and cause inflammation and gingivitis. If bacteria are not removed and the inflammation continues, the gum tissues can be destroyed and more advanced stages of gum disease may follow. Learn more about gum disease. To keep your teeth As gum disease advances, the pockets grow deeper, and plaque moves further down the tooth root, destroying supporting bone. The affected teeth may loosen and eventually fall out. Since gum disease is the leading cause of tooth loss in adults, regular dental exams and cleanings and brushing and flossing in between visits are vital to keeping your teeth.   Root Canals

  A “root canal” is a treatment of the pulp of the tooth that is inflamed, infected, or dead. The pulp is a soft substance in the center of the tooth that consists of nerve, blood vessels, and connective tissue. The pulp chamber is the hollow part in the center of the tooth that contains the pulp, and it continues down canals that extend through the roots of teeth and into the surrounding bone. During a root canal procedure, the nerve and pulp is removed and the inside of the tooth is cleaned and sealed. Root canal procedures save teeth. Millions of teeth are treated and saved each year with root canal. To understand a root canal procedure, it helps to know about the anatomy of the tooth. Inside the tooth, under the white enamel and a hard layer called the dentin, is a soft tissue called the pulp. The pulp contains blood vessels, nerves and connective tissue, and helps to grow the root of your tooth during development. There are many things that can damage the pulp or nerve of the tooth. Infection of the pulp can be caused by trauma to the tooth, deep decay, cracks and chips, or repeated dental procedures. Symptoms of the infection can be identified as visible injury or swelling of the tooth, sensitivity to temperature or pain in the tooth and gums. Endodontic treatment is necessary when the pulp becomes inflamed or infected. During root canal treatment, the inflamed or infected pulp is removed and the inside of the tooth is carefully cleaned and disinfected, then filled and sealed with a rubber-like material called gutta-percha. Afterwards, the tooth is restored with a crown or filling for protection. After restoration, the tooth continues to function like any other tooth.

  Fillings

  A filling is a way to restore a tooth damaged by decay back to its normal function and shape. Decayed tooth are usually treated by removing the decayed portion and then “fill” the area on the tooth where the decayed material was removed with filling material. By closing off spaces where bacteria can enter, a filling also helps prevent further decay. Fillings are also used to repair cracked or broken teeth and teeth that have been worn down from misuse (such as from nail-biting or tooth grinding). There are a variety of different materials that can be used for filling teeth. No one type of filling is best for everyone. What’s right for you will be determined by the extent of the repair, whether you have allergies to certain materials, where in your mouth the filling is needed, and the cost. Considerations for different materials include: Gold fillings are made to order in a laboratory and then cemented into place. Gold inlays are well tolerated by gum tissues, and may last more than 20 years. For these reasons, many authorities consider gold the best filling material. However, it is often the most expensive choice and requires multiple visits. Amalgam fillings are more commonly known as silver or mercury fillings. They are made up of silver, tin, copper, zinc, and mercury. They are resistant to wear and relatively inexpensive. However, due to their dark color, they are more noticeable than porcelain or composite restorations and are not usually used in very visible areas, such as front teeth. Composite resin fillings are more commonly known as white fillings, tooth-colored fillings, and direct veneers. They are made up of very tiny pieces of silica surrounded by a plastic resin usually composed of bis-GMA. They are matched to be the same color as your teeth and therefore used where a natural appearance is desired. The ingredients are mixed and placed directly into the cavity, where they harden. Composites may not be the ideal material for large fillings as they may chip or wear over time. Porcelain fillings are called inlays or onlays and are produced to order in a lab and then bonded to the tooth. They can be matched to the color of the tooth and resist staining. A porcelain restoration generally covers most of the tooth. Their cost is similar to gold.

• Night Guards

  What are Night Guards? These small pieces of plastic are designed to fit securely around your teeth. They are actually quite small and can fit in the palm of your hand. Typically, they are made of clear plastic, so they will not stand out when you wear them. This is excellent news for those who are self-conscious about appearance but want to save their teeth. To see what one looks like, visit our dental office. Do I really need to wear a night guard? If you are grinding your teeth at night – yes, you do. At Dr. Dena’s Encinitas office, we believe in the value of preventative care, and night guards are one of the ways we can prevent you from damaging your teeth. Since you only get one set of permanent teeth, allowing them to be ground down is unwise. The discomfort you will suffer and the cost you will incur to repair your teeth is simply not worth skipping out on this easy, preventative measure. We know that not all of our patients enjoy the idea of wearing a guard, and we will not recommend doing so unless we are certain that it will be useful in solving your problems. WHY DO MOST PEOPLE WEAR A NIGHT GUARD? Prevent damage. If you have paid to repair your teeth or have had cosmetic work done to improve your appearance, teeth grinding could reverse all that the work. Even if your teeth have never needed repair, grinding can cause your teeth to chip, crack, or simply wear down. This translates into direct discomfort as the teeth flatten, and your dentin or nerves could become exposed. This can lead to feelings of sensitivity when you eat or drink. If this type of damage continues, you could end up with gum recession and even loose teeth. If you use a night guard, your upper and lower teeth will be unable to grind against each other and the potential damage will be avoided. Ease discomfort. Nighttime teeth grinding can cause severe headaches and tooth pain in the morning. If you are regularly waking up in pain or feeling uncomfortable, this can be a sign that you need to wear a guard while sleeping. At Dr. Dena’s Encinitas office, we have found that many of our patients feel significantly better after wearing their guard for just a few days. The key is to continue wearing it every night. While some grinding is caused by stress or tension, there is no way to tell exactly what causes people to grind their teeth or to stop the body from attempting it. This is why continued use of the guard is necessary. Treating TMJ. Severe grinding or jaw clenching can lead to TMJ disorder. This can be incredibly uncomfortable, typically starting out as a sore jaw in the morning with the occasional headache. If you experience these symptoms or have face pain, neck pain, or your jaw hurts while eating, call 760-943-7788 to schedule an appointment with Dr. Dena right away. It is critical that you treat TMJ early by wearing a night guard and preventing your teeth from clenching or grinding in your sleep. Otherwise, TMJ can become severe. At this stage, some people find it difficult to even open their mouth, let alone eat or speak normally. In some instances, you may need to be referred to a TMJ therapist. What should I do to take care of my night guard? Another way to keep your guard in good shape is to brush it regularly. Cleaning it is relatively easy. All you need to do is brush it with a soft toothbrush, toothpaste, and lukewarm water. This should remove the plaque that has built up on it. Additionally, you can try a special dental solution and soak it once per week. Be sure to dry it afterwards to prevent any bacteria buildup.

• Deep Cleaning

  What is Deep Cleaning in Encinitas (Deep Scaling & Root Planing)? You may notice that your gums bleed a bit when you are brushing or flossing. This is the early stage of gum disease called gingivitis. If plaque and tartar are left on the teeth, it provides the right conditions for bacteria to thrive. The bacteria irritates the gums, which means they bleed more easily. If you have gingivitis, your dentist will clean your teeth by scaling and polishing them. He may also recommend an antiseptic mouthwash containing chlorhexidine and show you how to brush and floss your teeth effectively. Most adults have some degree of gum disease. Should I be concerned with my dental health? If gingivitis is not treated and nothing is done about it, the inflammation will work its way down towards the foundations of the tooth causing a “periodontal pocket”. The bacteria inside the pocket will multiply causing infection that in turn deteriorates the bone surrounding the teeth. Periodontal (Gum) disease can break down the support (bone) structures of the teeth, so that eventually, they will become loose. The problem is that until it gets quite severe, the person often has no symptoms. Sadly, the damage or bone loss to the teeth is irreversible. The good news is that if periodontal (gum) disease is caught in time, its progression can be halted and stabilized, the active infection is reduced or eliminated. To stop periodontal (gum) disease from progressing, your dentist may advise periodontal therapy, or deep cleaning. This gets rid of the bacteria in the pocket and provides the necessary conditions for healing to occur. What is the difference between a regular cleaning and a deep cleaning? Regular dental cleaning is the process of removing plaque and tarter that is above the gums and deep cleaning is the process of removing plaque, tarter and calculus (hard form of plaque) that is below the gums. Does it hurt? Depending on the depth of the pocket and severity of the gum disease, Dr. Dena may make the area numb so that the process is comfortable for you. Don’t hesitate to discuss with Dr. Dena how to best manage any discomfort. Sometimes if the pockets are not too deep, there may be little or no discomfort during the procedure – even without numbing. The only sensation may be the physical scraping feeling along the teeth as the area is cleaned and smoothened. Dr. Dena uses an Ultrasonic Scaler, which is more effective and comfortable for the patient, in addition to hand instrumentation. A tooth that is free of tartar has a better chance of allowing the gum tissues to heal and reattach to it. As a result, some deep gum pockets can be reduced after a deep cleaning. How long does it take? Typically with deeper pockets, the deep scaling and root planing procedure might be broken down into two appointments. For example, the right side of the mouth might be worked on one day, and the left side at a separate appointment. However on some cases Dr. Dena may be able to perform the deep cleaning in one appointment. Dr. Dena may use antibiotic gels within the periodontal pocket to decrease bacteria and may irrigate the pocket with various medications such as chlorohexidine. What can I expect afterwards? Discomfort can vary after root planing, but one can expect it to be tender afterwards since it’s usually in a deeper region under the gums. The teeth themselves can become a bit more sensitive to temperature, and bleeding might occur for a little while. Over-the-counter painkillers such as Tylenol or Ibuprofen work very well to alleviate discomfort Brushing and flossing can be done more gently to avoid aggravating any bruised or tender gum areas. Your dentist may recommend salt water or chlorhexidine rinses. Generally though as health returns to your gums, sensitivity and soreness tend to resolve. Remember though, if diagnosed with periodontal disease, you did not get there overnight and we cannot treat it overnight. If you are faithful with your home care it usually will resolve. Successful treatment is 70% what you do at home to treat the disease and 30% what we do when you come in for treatments. Left alone it will only get worse. If you go through the non-surgical therapy and do not continue with the maintenance therapy the disease will come back again. Periodontal Disease The objective of scaling & root planing is to remove etiologic agents which cause inflammation to the gingival (gum) tissue and surrounding bone. Common etiologic agents removed by this conventional periodontal therapy include dental plaque and tartar (calculus). These non-surgical procedures which completely cleanse the periodontium, work very effectively for individuals suffering from gingivitis (mild gum inflammation) and moderate/severe periodontal disease. Reasons for scaling and root planing Scaling and root planing can be used both as a preventative measure and as a stand-alone treatment.

  White Fillings

  What is a White Filling? For those with dental work who like to talk, laugh or smile, a silver tooth filling shows the world where we could have taken better care of our teeth. But what if there was a way to hide the fact that you even had any cavities in the first place? A composite filling is a tooth-colored plastic and glass mixture used to restore decayed teeth. Composites are also used for cosmetic improvements of the smile by changing the color of the teeth or reshaping disfigured teeth. The Makeup of a Tooth The external part of the tooth is called the enamel, which is arguably the hardest natural material produced by animals. It consists of very closely knit calcium crystals and contains no living tissue. Dental porcelains easily and efficiently imitate the enamel part of a tooth. The inner part of a tooth is the dentin, which is more porous and similar to bone. It consists of microscopic tubes of collagen with calcium crystal deposits. Dentin is living and very sensitive. Feeling in the dentin is transmitted through the pulp, or nerve tissues, in the very center of the tooth. To mimic the natural dentin in the teeth, today’s dental composite resins are common in non-amalgam fillings. These composites are made of a plastic resin and silica filler. What are the advantages of white fillings? Aesthetics are the main advantage of composites, since we can blend shades to create a color nearly identical to that of the actual tooth. Composites bond to the tooth to support the remaining tooth structure, which helps to prevent breakage and insulate the tooth from excessive temperature changes. They require very little tooth structure to be removed for placement which preserves more of your natural tooth bonded to your tooth so they won’t fall out. What are the disadvantages? After receiving a composite, a patient may experience postoperative sensitivity. Also, the shade of the composite can change slightly if the patient drinks tea, coffee or other staining foods. The dentist can put a clear plastic coating over the composite to prevent the color from changing if a patient is particularly concerned about tooth color. Composites tend to wear out sooner than silver fillings in larger cavities, although they hold up as well in small cavities.

  An onlay restoration is a custom made filling made of composite material, gold, or tooth-colored porcelain. Porcelain onlays are popular because they resemble your natural tooth. An onlay is sometimes also referred to as a partial crown. Porcelain onlays are made by a professional dental laboratory and is permanently cemented onto the tooth by your dentist. Onlays can be utilized to conservatively repair teeth that have large defective fillings or have been damaged by decay or trauma. Onlays are an ideal alternative to crowns (caps) because less tooth structure is removed in the preparation of onlays. Onlays are essentially identical to inlays with the exception that one or more of the chewing cusps have also been affected and need to be included in the restoration. As with most dental restorations, onlays are not always permanent and may someday require replacement. They are highly durable and will last many years, giving you a beautiful long lasting smile. An onlay procedure usually requires two appointments. While the tooth is numb, the dentist will remove any decay and/or old filling materials. The space will then be thoroughly cleaned and carefully prepared, shaping the surface to properly fit an onlay restoration.
